Position Description
Under the direction of the Case Manager Supervisor, the Coordinator will help navigate community programs and resources related to three main issues; Homeless, Suicide Prevention and Justice Involved Veterans. This role will be relied upon to successfully reintegrate Veterans, assist in organization development, program measurements and ensure standards of excellence as the role is forward facing within public and community settings.
This position will be instrumental in working with existing programs that provide and promote the social and economic well-being of military Veterans by providing resources and information that specifically make a difference.
Duties and Responsibilities:
Conducts outreach efforts in the streets, jails, hospitals and rehabilitation centers to find Veterans in need of assistance, guidance and mentorship
Builds a rapport with internal and external organizations, which we can build effective collaborations in getting the best possible outcomes for our Veterans
Works with other supervisors and the Deputy Director to identify and resolve existing or potential problems to ensure a productive and effective work environment
Receives training by the staff and community, to remain in the forefront of program development of effective programs within our community, in which we can coordinate partnerships to help Harris County Veterans
Serves as an advocate and peer, working alongside our Veterans, therefore, knowledge of our Veteran community is essential; issues, concerns, traumas, family concerns, organizations, etc.
Works with a variety of programs, including, but not limited to; justice system, homeless, medical facilities, emergency financial assistance, case management, bereavement services, intake services, etc.

Requirements
High school diploma or GED equivalent
A minimum of two (2) years case management experience
Must be self-motivated
Ability to build mutual respect, develop effective relationships and work effectively with co-workers and administrative personnel, but also, the community at large, specifically Veterans and Veteran family members
Ability to control volatile situations in a professional manner and recognize areas of concern to address the potential for adjustment or chance
Organized, conscientious work ethic
Ability to learn and apply policies and procedures pertinent to the division and the agency

Preferences
Bachelor’s Degree (Social Work, Counseling, etc.) from an accredited college or university

HOURS:
Monday - Friday / 8:00 AM - 5:00 PM
SALARY:
Commensurate with Experience
Based on 26 Pay Periods
